Overview

The aim of this survey is to look at the impact of Learning Disability services in Luton on the lives of children and young people with a learning disability and their families. This questionnaire will form part of the action plan to improve services.

Why We Are Consulting

In order to see how good the services are, we need to talk to people who use them. This survey is designed for parents and carers of people with a learning disability, to get their views and experiences. Your feedback is important to us. Some questions may only apply to either adults (18 -25 years old) or children (0 -18 years old) – this is indicated in the survey.

Your name and contact information will not be collected. All responses must be received by 31 March 2020. The survey forms wil be destroyed six months after the surevy ends.

We're committed to protecting your privacy when you use our services. We'll make sure we hold records about you (on paper and electronically) in a secure way, and we’ll only make them available to those who have a right to see them.
